This service supports component districts in providing services aligned with becoming a community school. The purpose of this service is to coordinate cooperative and proactive inter-agency service delivery systems. Services that may be offered through this Co-Ser include:

• District Needs Assessment related to student needs and organization systems
• Service Plan development based on identified and documented needs
• Implementation of Academic Services
• Consultation and Support for district staff
• Coordination of services, outreach, and relationship building with providers
• Universal student/family assessment tool to identify needed services and referral process
• Establishment and regular meetings of a Partner Advisory Committee comprised of the program, district, community agencies, and other stakeholders
• Customized professional development days
• Technical Assistance
• Ongoing progress monitoring, data collection, analysis and reporting

The Co-Ser will provide an integrated, comprehensive service delivery system that promotes the physical, mental, emotional, and behavioral health of students by implementing appropriate supports and services that enable learners to excel through aidable interagency partnerships. Examples include mental health services, telemedicine, school physician or medical director, and alcohol and substance abuse recovery systems.
