Pre-Planning Question Packets

This questionnaire should be completed by students prior to their pre-planning meeting. This questionnaire is meant to initiate thought and discussion about students’ goals and plans for the future. It is also intended to assist students in preparing for their pre-planning and IEP meetings.

  • Agency Connection Letter (This letter is sent home prior to beginning the pre-planning process in order to gather information about which agencies are working with a student and should be included in the pre-planning meeting.)
  • Invitation Checklist (This is a checklist of possible people to invite to a pre-planning meeting.)
  • Transition Pre-planning Parent Meeting Notice (This is the invitation letter sent to parents to schedule a time for the pre-planning meeting.)
  • Transition Pre-planning Agency Meeting Notice (This is the invitation letter sent to any Agency Representatives working with the student or to representatives who may work with the student in the future. This invitation is also sent to district CSE Chairpersons and Teachers that work with the student.)
  • Pre-planning Worksheet (This is the form used to record the information during the pre-planning meeting.)
  • Pre-planning Signature & Goals (This form should be printed prior to Pre-planning meetings. It will be used to draft IEP goals. This forms also requires signatures
